- Have you tested positive for Covid-19 or have you been told by a medical professional that you are presumed positive?
- Do you currently have symptoms associated with Covid-19, such as fever, shortness of breath, cough, sore throat or other flu-like symptoms? (If you have any of these symptoms we recommend you consult with your doctor).
- Have you been in contact with anyone who has tested positive for Covid-19; or someone a medical professional indicated was presumed positive in the last 14 days?
If you need a refill, but our protocols are requiring a blood test (and you said “yes” to one of the questions above), then contact our team – we are happy to evaluate a refill exception.
